Occupational Therapist - Home Health, West Portland, OR

Overview
• Shift: Day 5x8-Hour (08:00 - 16:30)

Requirements
• License: Oregon
• Certs: BCLS
• Experience: 1 YR of Skilled Home Health Experience
• EMR: EPIC Remote Client, Rover, Oasis
• Compliance: Primary series COVID vaccination required, plus one booster
• Locals: Considered local within 50-75 mile radius
• RTO Policy: Confirm with hiring manager

Responsibilities
• Provide occupational therapy services to home health patients.
• Conduct evaluations and develop treatment plans based on patient needs.
• Educate patients and families regarding home health care techniques.
• Document patient care and treatment progress in the designated EMR system.
• Collaborate with interdisciplinary teams for comprehensive patient care.
• Ensure compliance with all home health regulations and standards.
