Physical Therapist - $60/hr - Glendale, AZ

Position Overview:

We are seeking a Physical Therapist to provide evaluations and therapy services for students in grades K-8. This role includes performing assessments, implementing therapy plans, and documenting services in accordance with physician orders and individualized plans of care.

Assignment Details:

  • Location: Glendale, AZ

  • Start Date: December 18, 2025

  • End Date: End of 2025-2026 School Year

  • Schedule: Monday-Friday, 40 hours/week

  • Duration: 36 weeks

  • Pay Rate: $60/hour

  • Modality: In-person

  • Caseload: Split between 2-3 campuses (all within 15-20 minutes of district office)

Responsibilities:

  • Conduct evaluations and develop individualized therapy plans

  • Provide physical therapy services in accordance with physician orders

  • Document therapy sessions and maintain compliance with district and state guidelines

  • Collaborate with teachers and multidisciplinary teams

Requirements:

  • Current Arizona Physical Therapist license

  • Graduate of an accredited Physical Therapy program

  • One year of prior professional PT experience preferred

  • Must meet all federal, state, and local requirements

  • Current CPR, TB questionnaire, and health certificate if applicable

  • Must be at least 18 years of age
