Join our dedicated team as a Care Manager RN This is a Full-time position working 8-hour Day shifts from Sunday to Thursday.

As a key player in Case Management, you will collaborate with patients, families, nurses, social workers, physicians, payers, and other healthcare professionals to facilitate seamless communication and care coordination across various settings, recognizing the importance of patient autonomy and the social determinants of health.

Required Qualifications:
- Bachelor's Degree or equivalent education/experience.
- California Registered Nurse License upon hire.
- Minimum of 2 years’ experience in a Healthcare setting (Acute, Ambulatory, Post-Acute, etc.).
- Minimum of 2 years’ experience in Case Management (Care Coordination or Utilization Management) or completion of the Transitions in Practice (TIP) program for Care Managers.
Preferred Qualifications:
- Bachelor's Degree in Nursing.
- Master's Degree in a Healthcare-related field.
- Certification in Case Management (ACM or CCM) upon hire.

The salary range for this position is $56.12 - $84.75, with additional compensation opportunities, including shift differentials and bonus incentives.
