MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
2. Valid driver's license.

1. 2 years of healthcare experience

1. The Rehab Liaison is responsible for cultivating referral relationships. This role is crucial for driving growth through patient referrals and involves developing strong relationships with various stakeholders, including patients, family members, physicians, and healthcare professionals

2. The rehab liaison is expected to exercise sound judgment in handling the professional and confidential nature of healthcare sales, ensuring that you maintain the highest standards of integrity and professionalism in all interactions

3. The position blends professional growth with community connection, emphasizing the importance of collaboration and communication in the rehabilitation process

4. Educate community, referral sources, and physicians on unit programs and services

5. Respond to and overcome admission barriers and follow up on admissions variables

6. Conduct in-services and professional presentations to various groups about services and outcomes

7. Clinical Liaisons perform clinical assessments of referred patients, prioritizing face-to-face connections

8. Educate referred patients and families on acute rehabilitation programs

9. Aid in streamlining the referral-to-admission process

10. Liaisons partner with hospital case managers to create smooth patient transfer processes and to help remove payer-related and operational barriers to getting appropriate patients into the best next level of care

11. Work with hospital case managers and physicians to identify and transition patients into the next level of care:

12. Develop and cultivate professional relationships with case managers, physicians, and clinicians in acute care hospitals

13. Manage the referral process effectively to ensure timely admissions

14. Accurately and efficiently complete the Pre-Admission Screen adhering to all policy, regulatory and payor guidelines.

15. Maintain knowledge of CMS and third-party payor Inpatient rehabilitation requirements

16. Demonstrate excellent customer service skills with focus on referrer satisfaction and account retention

17. Develop strategies to grow market share in core business lines.

18. Develop business census based on strategic goals

19. Utilize market analysis to identify new opportunities

20. Creates quarterly goals and objectives and submits to supervisor

21. Works with Director of unit and medical Director to identify opportunities for expansion of strategic business initiatives

1. The National Institute for Occupational Safety and Health recommends minimizing the need for employees to manually lift patients and limit safe patient handling to 35 pounds. All transfers, lifts and re-positioning will be completed utilizing the lift equipment and/or other patient handling aids as indicated by the patient's profile and appropriate algorithms.

2. Heavy/Hard work: Work requires strength and/or stamina, lifting, moving, stooping, reaching, standing, walking, and carrying of materials and equipment weighing 40+lbs.

3. Visual acuity must be within normal range. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.

4. Hearing within normal range is required

SKILLS AND ABILITIES:

1. Skill in organizational matters, including time management, prioritization, multitasking, and problem-solving.

3. Ability to use excellent customer service, written and oral communication skills.

4. Excellent communication and interpersonal skills to build relationships, educate patients and families, and collaborate with the healthcare team.

5. Ability to navigate an electronic health record efficiently. Basic knowledge of computer programs such as work processing, spreadsheet, and email applications.

6. A strong clinical background, often as a registered nurse or therapist, is essential for understanding patient needs, exercising outstanding clinical judgment, and assessing their suitability for rehabilitation.

7. Understanding healthcare regulations, insurance requirements, and the intricacies of managed care is crucial for navigating the admission process.
