Join us and enjoy competitive compensation and benefits while making a difference in the community About the Role In this role, you will conduct comprehensive health assessments in members' homes or Skilled Nursing Facilities, with visits lasting up to one hour. Our providers: Conduct adult/geriatric assessments, reviewing medical history, diagnosing, offering treatment, providing health education, making physician and case management referrals, and documenting according to Matrix guidelines. Administer and order necessary point-of-care and lab-based testing. Collaborate with physicians, case managers, social workers, family members, and other medical personnel as appropriate. Work alongside Primary Care Physicians for patient education and follow-up. Benefits of Working at Matrix Consistency: Full-time providers typically work Monday to Friday, 8:00 to 5:30, covering assessments from Sunday to Saturday, 8 am to 7 pm. Competitive Pay: Earn an annual base salary ranging from $95,500 - $158,400 based on market analysis and experience. Incentive Program: Enjoy monthly (uncapped) and year-end incentive opportunities. Licensure Support: We will assist you in obtaining licensure in neighboring states as needed for assessments. Qualifications Master's Degree or equivalent experience and completion of NP licensure. Current NP licensure in the state of practice with prescription authority or ability to obtain such authority. AANP, AACN, or ANCC certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care specialties. Current BLS, ACLS, or CPR certification. 1 year of NP experience is preferred; new graduates are encouraged to apply. Willingness to travel up to 15% of the time, including overnight trips. Valid state driver's license and proof of adequate automobile insurance. Proficient in computer skills and familiarity with employee health/medical record software.
