Position Overview: The Community Liaison is a pivotal role that bridges our healthcare services with the community. This position focuses on business development and manages communications with patients, caregivers, and referral partners while championing our mission across various market segments.
Key Responsibilities:
- Business Development & Outreach:
- Identify and implement in-services within the healthcare community.
- Establish and nurture strong partnerships with referral sources, including hospitals and senior living facilities.
- Seek new opportunities to enhance patient numbers across various service lines.
- Represent the organization at community events, health fairs, and professional gatherings.
- Patient & Family Engagement:
- Educate families and caregivers about the benefits of hospice services.
- Act as a compassionate resource to assist families through the enrollment process.
- Communication & Coordination:
- Develop connections with healthcare staff and facilitate effective communication between referrals and internal teams.
- Ensure timely follow-ups on referrals and maintain accurate outreach documentation.
- Market Leadership:
- Provide educational opportunities for healthcare professionals on hospice services.
- Analyze referral patterns to identify and address community needs.
- Maintain an understanding of local healthcare markets to support outreach efforts.
- Serve as the organization's representative, building brand awareness and trust.
- Monitor market trends to inform strategic initiatives.
Required Skills and Experience:
- Experience in healthcare marketing, sales, or community outreach, preferably in home health or hospice.
- Proven ability to achieve performance goals and key metrics, including referral volume and patient admissions.
- Strong communication and interpersonal skills to engage diverse audiences.
- Familiarity with Medicare/Medicaid regulations is beneficial.
- Knowledge of hospice eligibility and care levels is preferred.
- Experience with CRM systems for tracking engagement activities.
- A self-starter with the capacity to collaborate in a fast-paced environment.
- A valid driver's license and reliable transportation are required.
Preferred candidates will demonstrate a background in using data to enhance outreach effectiveness and contribute to overall business growth.
