EIHC is seeking a full-time Care Manager dedicated to serving the needs of our community.

SCHEDULE

Monday through Friday, flexible scheduling my be possible

COMPENSATION

$28.00 to $45.00 hourly depending on experience

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Support and empower patients throughout the health care continuum using whole person coaching and problem solving as needed on medication adherence, disease education, and self-help behaviors.
  • Complete assessments of the barriers to chronic disease management that contribute to health disparities and identify information and support for medically underserved patients.
  • Participate and collaborate in opportunities related to community outreach, increasing cancer screening rates, and partnerships for chronic disease prevention.
  • Facilitate communication and coordination between members of the health care team, including social workers for the appropriate use of health care services.
  • Demonstrate knowledge of multiple chronic diseases and interventions and engage the patient and patient's family in appropriate self-management goals.
  • Oversee utilization of population and disease management tools to improve the health and wellness of EIHC's patient population, specifically those with chronic diseases such as hypertension, diabetes and COPD.
  • Will include cross coverage into all departments.

REQUIRED QUALIFICATIONS
  • Current LPN or RN license in the State of Iowa required.
  • Bachelor of Science in Nursing preferred.
  • Previous experience in chronic disease management and quality assurance preferred.
  • 5+ years LPN or 2+ years RN experience required, FQHC experience preferred.
  • Previous experience health coaching or completion of health coach competency within one year of hire date preferred.
