As the Associate Clinical Center Director, you will play a pivotal role by supervising and training primary care providers (PCPs) within your designated center. Your focus will be to enhance clinical quality, efficiency, and patient satisfaction while also providing direct patient care. Your leadership will guide our dedicated team as we strive for excellence in medical care.
Key Responsibilities:
- Deliver comprehensive patient care to older adults with acute and chronic conditions.
- Take full responsibility for patient outcomes while collaborating with specialists when necessary.
- Engage with hospitalists to coordinate care for hospitalized patients.
- Oversee assessments, diagnoses, treatments, and care management, ensuring holistic patient education and health promotion.
- Lead your care team—comprising medical assistants, care facilitators, and care coordinators—to foster effective patient interactions.
- Support patients unable to visit the office by connecting with transitional care teams and resources.
- Facilitate weekly Super Huddles and Transforming Care Meetings, ensuring smooth center operations.
- Actively manage the center alongside other providers, contributing to patient recruitment and the hiring of additional medical staff.
- Mentor and coach PCPs to achieve outstanding clinical outcomes.
- Collaborate with the Center Operations Director to drive improvements in financial performance and membership growth.
- Perform any additional duties as directed by management.
Required Skills and Qualifications:
- Proficiency in Microsoft Office Suite and other relevant applications.
- Willingness to travel locally and occasionally for recruitment and training.
- Fluent in English, both spoken and written; other languages may be advantageous.
- Ability to exercise independent judgment in patient care and team leadership.
Education & Experience:
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
- Active medical licensure in your state of employment.
- A minimum of 1 year of clinical experience in geriatric, adult, or family practice settings preferred.
- Completion of ChenMed training, including Masterful Conversations.
- Board certification in a relevant specialty is preferred; board eligibility is required.
- Current DEA registration for controlled substances.
- BLS certification from an accredited provider within 90 days of employment.
