Your Role:

As a PRN Home Health LPN, you will provide essential nursing care, which includes:

  • Administering wound care and medications in a home setting.
  • Documenting patient assessments and care interventions accurately.
  • Monitoring patient health and communicating changes to the care team.
  • Educating patients and their families to foster independence and understanding of care processes.
  • Collaborating with other healthcare professionals to ensure personalized care plans are followed effectively.

Qualifications:

  • Current LPN/LVN licensure.
  • Valid Driver License.
  • Graduate of a Vocational Nursing Program.
  • Minimum of 1 year of nursing experience in a clinical or home health setting.
