Job Title: Home Health RN Profession: RN Home Health Specialty: RN Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum 1 year of Certified Home Care experience, 1 year of Case Management experience License: Active RN License Certifications: None specified Must-Have: Ability to travel to assigned cases, Weekend rotations every 6 weeks Description: All applicants must have 1 year of Certified Home Care experience. Applicants must have 1 year of Case Management experience. The role requires the ability to travel to cases as assigned. Weekend rotations occur once every 6 weeks. As a Home Health RN, responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within the organizational philosophy, policy, and standards of community health practice. The role also involves performing admission processes and case management responsibilities according to policies and standards of practice. Thorough and timely electronic documentation and interdisciplinary communications are required.
