Responsibilities:

  • Provide family support services that enhance family functioning and child well-being.
  • Coordinate with community providers to facilitate comprehensive care and referrals for families.
  • Engage in collaborative case planning and case review meetings as required.
  • Conduct case management activities including:
    • Supervised visitation
    • Parent aide support
    • Behavioral aide assistance
    • Transportation services
    • Crisis intervention
    • Advocacy and linkage to services
  • Complete in-home visits to monitor progress and provide ongoing support for families involved with DFCS.
  • Offer parenting education and model positive parenting strategies.
  • Work closely with external professionals (e.g., schools, therapists) for service alignment.
  • Maintain accurate documentation of services rendered per agency expectations.
  • Provide transportation services related to the family service plan as appropriate.

Qualifications:

  • Bachelor's degree in a human services-related field (e.g., Social Work, Psychology, Counseling, Criminal Justice) or a High School Diploma/GED with at least 5 years of verifiable experience in the field.
  • Strong understanding of family systems, trauma-informed care, and community resources.
  • Reliable transportation, valid driver's license, and proof of insurance.
  • Willingness to complete required background checks and contractor onboarding.

Deliverables:

  • Timely submission of service logs, progress updates, and reports.
  • Accurate documentation adhering to billing and reporting standards.
  • Provision of services in accordance with contract terms, including hourly or task-based invoicing.
