Job Title: Medical Coordination Specialist
Reports To: Residential Manager
Primary Purpose of Job
A Medical Coordination Specialist is responsible for providing medical coordination, support, guidance and assistance to people with intellectual and developmental disabilities within a Residential site. In this role will play an integral part in enabling people supported at ACLD to have a quality life, an enviable life. An advocate for the people supported in communicating their needs and goals. Will actively promote the health, safety and well-being of the people supported by providing hands-on assistance, mentoring and teaching independent living skills and providing services as directed in their Life Plan.
Requirements
- High School Diploma or equivalent required.
- Clean, valid New York State driver’s license.
- Ability to communicate and understand in English; both verbally and in writing.

The ability to obtain (within 90 days of hire) and maintain AMAP (Approved Medication Administration Personnel) Certification.

The ability to obtain (within 180 days of hire) and maintain SCIP (Strategies for Crisis Intervention and Prevention) Core Certification, CPR (Cardio-Pulmonary Resuscitation), First Aid and AED (Automated External Defibrillator) Certification.

One (1) year experience with developmentally disabled population preferred.

The ability to assist with lifting and transferring a person. The ability to lift up to 15 lbs.

The ability to reach, bend, twist, kneel, and squat as you assist people with physical activities.

The ability to perform SCIP maneuvers which may require bending, twisting, kneeling, crouching, lying on the ground, and supporting another person’s body weight.
- The need for SCIP and/or AMAP certification will depend on program needs. Some staff may be required to attend the SCIP Restrictive course instead of the SCIP Core course.
Additional Knowledge, Skills and Ability
• Substantial reasoning skills required including the ability to deal with a variety of abstract and concrete issues simultaneously; the ability to identify problems, interpret data and draw conclusions; and the ability to apply principles of logic or theoretical thinking to a wide range of intellectual and practical problems.
• Excellent verbal and written communication skills including the ability to coach and counsel, interpret and advise, and the ability to professionally assess and resolve problems.
• Ability to perform basic computer skills preferred.
Ability to provide clear and legible documentation on records.
Essential Functions
• Oversee the dispensing and documentation of medication in accordance with AMAP procedures and follow all AMAP guidelines with regards to conducting medical appointments as assigned.
• Maintain appropriate stock of medications for each person supported, including OTC, etc.
• Meet with the residential nurse to conduct weekly nursing coordination.
• Schedule all medical appointment follow ups as directed.

• Attend medical appointments with people supported, schedule follow up, collect all paperwork, submit to nurse for review.
• Communicate with families regarding medical appointments as needed.
• Prepare the MARs on a monthly basis .
• Review all MARs and corresponding documents for accuracy.
• Ensure refills of medications are ordered in a timely manner to prevent out of stock issues.
• Ensure new prescriptions are obtained in a timely manner as per procedure
• Ensure all documents are prepared and accompany person supported on medical appointments (medical treatment summaries, diet orders, prescriptions requests, lab orders, Nurse’s notes, etc.)
• Complete medication administration error reports as needed
• Respect the human dignity and uniqueness of the people you support. Recognize each person you support is valuable and advocate their value to others.
• Implement and document the delivery of all Residential Habilitation services which may include but is not limited to: person supported skill development training in a variety of areas including: medical concerns, vocational concerns and recreational services.
• Implementation of all person supported behavioral support plans/strategies and documentation of person supported behaviors.
- Support the emotional, physical, and personal well-being of the people you support. Encourage growth and recognize the autonomy of the people you support while being attentive in reducing their risk of harm.
• Provide personal care assistance to ensure maximum level of supervision and to promote the highest level of health, safety and independence.
• Assist people supported in performing daily living tasks, including, but not limited to, room cleaning and household chores. The staff will be required to perform housekeeping duties within the program.
• Ensure that program environments meet the highest quality standards as per OPWDD guidelines and report any concerns to supervisor.
• Assist people supported in food preparation; including preparing meal plans and shopping for supplies.
• Adhere to Agency systems designed to promote organizational efficiencies as well as compliance with ACLD policies, procedures and regulations governing programs.
• Supervise people supported within the program and community. Intensiveness of the supervision determined by the specific needs of the people supported.
• Complete and review assigned reports, logs, and documentation in accordance with ACLD Corporate Compliance Policy.
• Assist the people supported with the use of their personal allowance, including oversight in shopping, budgeting and managing their income.
Interact positively with families, Care Managers and other outside providers while assisting the people you support in developing and maintaining relationships. Represent ACLD in a positive manner.
• Assist the people supported with transportation utilizing agency vehicles.
• Implement SCIP (Strategies for Crisis Intervention and Prevention) maneuvers when necessary.
• Attend required staff meetings, supervision sessions and training programs as assigned.
• Maintain collaborative relationships with other ACLD programs and departments to maximize service coordination and integration.
• All other duties as assigned.
