JOB SUMMARY:
Working as a Caregiver in our Community Based Care Facility, you will assist residents with daily activities, promoting independence, and ensuring their safety and well-being, while also providing companionship. Caregivers work closely with residents, their families, and care team to provide personalized care that meets each individual's unique needs and preferences.
RESPONSIBILITIES:
- Provide assistance with activities of daily living (ADLs)
- Monitor and report any changes in residents physical or mental health
- Assists with serving resident meals
- Provide companionship, emotional support, and meaningful engagement to residents
- Assist in maintaining accurate records of care provided, including daily logs, progress notes, and incident reports
- A Caregiver will maintain resident confidentiality
- Encourage clients to participate in activities that promote physical, mental, and emotional well-being
- Support clients in maintaining independence in their daily routines and decision-making
- Ensure a safe environment by identifying and addressing potential hazards in community setting
- Respond promptly and appropriately to emergencies, such as falls, injuries, or sudden health changes, following established protocols
- Collaborate with the care team, including healthcare professionals, social workers, and family members, to provide coordinated care
- Communicate effectively with residents, families, and team members to ensure clarity and understanding of care plans and any changes
- Present for staff meetings and training
- Other duties as assigned
- Must be 18 years of age
- NAC/CNA or NAR or HCA acquired within 120 days of hire
- High school diploma or equivalent required
- Previous experience in caregiving, home care, or community-based care is preferred
- Ability to work independently and as part of a team
- Patience, empathy, and a genuine desire to help others
- Good organizational and time management skills
- Ability to perform physical tasks such as lifting, bending, and assisting residents with mobility
- Must be able to stand, walk, and reach for extended periods
- Ability to pass a background check
