About The Position
Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Physical Therapist Duration: 20 weeks Shift: M-F with weekend rotation Hours per Shift: Guaranteed 36 hours Experience: Minimum 1 year License: Vermont PT licensure Certifications: None specified Must-Have: Home health experience and OASIS experience, McKesson/Netsmart EMR experience preferred Description: Physical Therapist needed to provide one-on-one therapy care for patients in their home setting. Primary job responsibilities include assessing a person's condition and developing individualized plans of care to help individuals reach their goals and maximize mobility and independence. Collaboration with the interdisciplinary team and colleagues is essential to ensure the needs of the individual are met. Demonstration of the ability to perform standard therapy measurement techniques, including range of motion, strength, and balance tests, is required. Utilization of a variety of therapeutic modalities to maximize care provided to clients is expected.
