Qualifications:

  • A genuine passion for serving patients and families with exceptional care.
  • Education: Must be a graduate of an accredited master's, post-master's, or doctoral nursing program with formal training as a Nurse Practitioner.
  • Licensure: Must hold current unrestricted licensure as an Advanced Practice Registered Nurse (APRN) with Nurse Practitioner designation and full DEA privileges. Maintenance of national certification in the Nurse Practitioner specialty is required.
  • Experience: Applicants should have two years of experience in chronic disease management or long-term care within the last five years. A background in hospice and/or palliative care is preferred.
  • Physical Requirements: Must have reliable transportation and the ability to sit, stand, bend, lift, and move intermittently, with the capability to lift at least 80-100 lbs.
