We are seeking a Case Manager/Mental Health Counselor for The Doe Fund's Webster permanent supportive housing program for homeless men and women living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. Half of the residents will be referred by the New York City HIV/AIDS Services Administration (HASA), and the other half will be apartments for affordable to low-income households.
The Case Manager/Mental Health Counselor is responsible for providing supportive case management services to the HASA-funded residents. The successful candidate will have strong clinical and communication skills along with an understanding of working with the HIV/AIDS population.

The Ideal Candidate:
- You have a Master's Degree in Social Work or related field, a license in Clinical Social Work and LMSW or LMHC required.
- You have at least one year of experience working with people who are homeless, living with HIV/AIDS, have substance abuse histories, or suffer from mental illness
- You have at least 5 years of experience in the mental health field
- You have knowledge of benefits in the New York City Metro area, HIV/AIDS, psychiatric diagnoses, substance abuse, and mental health issues
- You have solid computer skills - internet, Microsoft Office, and charting database
- You have great organizational, writing, and communication skills
- Maintain a caseload and provide regular case management services to these individuals
- Maintain resident records in compliance with agency and funding standards
- Conduct and review assessments
- Develop and implement individual Service Plans and assist residents with achieving their goals geared towards self sufficiency
- Monitor and ensure residents are linked to healthcare providers
- Provide counseling and crisis intervention as necessary
- Assist residents in securing and maintaining benefits
- Maintain a liaison with community services including hospitals, substance abuse treatment programs, HRA and mental health programs
- Facilitate groups and activities that promote health and independent living skills development
- Monitor and assist residents with rent payments and budgeting
- Conduct regular unit inspections
- Educate residents about approaches to mental health
- Attend ongoing trainings to enhance the services provided to the residents
- Assist with completion of HASA's weekly and monthly reports as well as any other relevant reports
- Other duties as assigned
