LPN Case Manager Hours M–F, 8:00 am – 5:00 pm (Weekend on call rotation, Full Time) Counties Covered: Harrison, Crawford, Partial Floyd and Washington. LPN Case Manager Compensation The pay for this position is $34.30-$37.30 an hour Benefits are available to full-time employees A 401(k) with a company match is available for full-time employees. LPN Case Manager Responsibilities Provide care for clients following client's plan of care at all times while maintaining client's dignity and independence. Monitor client's vital statistics and report abnormal findings. Administer medications, feedings, oxygen, ostomy care as needed per client's plan of care and provide assessments of results. Complete routines as established by other care professionals such as PT, OT, Speech Therapists. Ensure infection control policies are maintained at all times, report possible communicable diseases or infections and ensure the overall implementation and practice of infection control procedures are implemented. Accurately document nursing actions of all care given and communication as appropriate as well as documenting the progress and outcomes for established goals and informs. Order supplies, medications, and feedings as needed to ensure adequate amounts are on hand at all times Ensure all patient needs are being met in accordance with the instructions of the physician. Monitor incidents involving individuals. Ensure incident reports are completed timely, accurately, and appropriate personnel notified; examine the individual following the incident. Ensure all medical equipment is maintained and report faulty equipment to DME for repair.. LPN Case Manager Requirements LPN License or Compact LPN License Preferred Skills: Wound Care, Drain care, G tubes, basic education. HCHB preferred.
