Job Title: Compassionate Caregiver - Make a Meaningful Impact in the Lives of Others

Location: Valleywide

We are seeking dedicated and compassionate caregivers to join our team in providing exceptional care and support to individuals in need. As a caregiver, you will have the opportunity to improve the quality of life for clients by assisting them with daily living activities and helping them maintain their independence in the comfort of their own homes.

In this role, you will implement personalized care plans and provide a variety of services, including companionship, meal preparation, light housekeeping, transportation, and assistance with personal care, such as bathing and dressing.

Qualifications:
  • Experience: A minimum of 1,000 hours of experience working with geriatric or chronically ill individuals is preferred. Exceptions may be made for recently licensed CNAs, HHAs, or those providing companionship and housekeeping services only.
  • Certifications: CNA or HHA certification is preferred but not required.
  • Health and Safety: Proof of current TB testing, ability to safely assist with client transfers, and adherence to infection control protocols.
  • Transportation: Reliable transportation, valid driver's license, and automobile insurance are required for client transportation.
  • Background Check: Must successfully pass a criminal background check (both national and state-required).
  • Communication Skills: Strong verbal and written communication skills for effective interaction with clients, family members, and team members.
Key Responsibilities:
  • Care Plan Implementation: Follow personalized care plans to ensure clients receive the necessary support and assistance.
  • Client Support: Provide companionship, assist with meal preparation, light housekeeping, transportation, and personal care (bathing, dressing, toileting, feeding).
  • Advocacy and Communication: Report any changes in client conditions to the Care Manager and advocate for the client's needs.
  • Safety and Compliance: Follow established safety protocols, infection control standards, and notify the office if additional supplies or resources are required.
  • Documentation and Timekeeping: Accurately document care tasks, hours worked, and client status using the ClearCare Telephony system.
  • Professionalism: Maintain a professional appearance and demeanor while adhering to company policies and procedures.
Physical Requirements:
  • Ability to assist with client transfers and repositioning using proper techniques and equipment.
  • Reliable transportation to clients' residences.
