Join Our Compassionate Hospice Team
We are looking for a dedicated Hospice Patient Care Manager to enhance our hospice services. In this crucial leadership role, you will report directly to our Executive Director or Administrator and oversee clinical operations to ensure that patients receive the highest quality of care possible. Your work will inspire our teams and uplift the spirits of those we serve.
Key Responsibilities:
- Oversee and direct patient care services to guarantee high-quality hospice care.
- Manage scheduling for patient visits, ensuring all decisions are made with clinical judgment and prioritization.
- Handle incoming phone referrals and medical concerns with compassion and professionalism.
- Lead interdisciplinary group meetings, provide updates on patient progress, and coordinate care plans.
- Act as a mentor and trainer, fostering development and accountability in our team.
- Ensure compliance with all regulatory standards and internal policies in patient care delivery.
- Drive performance improvement initiatives, enhancing quality and efficiency in our operations.
What You'll Bring:
- Current RN license in the state of employment.
- Graduation from an accredited nursing program.
- A minimum of 3 years of direct patient care experience, with at least 2 years in hospice or home care.
- A solid understanding of hospice principles, industry standards, and regulatory compliance.
- Compassionate approach to caring for terminally ill patients and their families.
- Proven ability to lead diverse teams while addressing the unique emotional and clinical needs of patients.
- Strong interpersonal, organizational, and leadership skills.
- Proficiency in clinical documentation and healthcare technology systems.
Preferred Qualifications:
- Bachelor of Science in Nursing (BSN).
- Previous experience in leadership or supervisory roles within hospice or home health settings.
- Familiarity with EMR systems and telehealth platforms.
- Certification in Hospice and Palliative Nursing (CHPN).
