  Knowledge and skills required for the position are:

  • Must have home health experience and HCHB experience including
  • HHRG score optimization and utilization. No hesitation to travel the region when needed.

• Monitor clinical documentation and ensure compliance with agency, state, and federal guidelines

• Participate in on-call rotation and assist with care coordination during non-business hours

• Partner with QA, intake, and scheduling teams to optimize patient outcomes and field operations

Qualifications

• Active RN license in the state of [Insert State]

• Minimum of 2 years of clinical home health experience; supervisory or managerial experience preferred

• Strong proficiency with Home Care Home Base (HCHB) EMR system

• Experience leading team meetings and analyzing clinical/operational data preferred

• Excellent communication, leadership, and problem-solving skills
