Job Title RN Home Health- Case Manager

Shift:

  • 8-hour Day Shift (M-F, 8:00 AM - 5:00 PM)
  • Every 4rd weekend rotation
  • No on-call or holiday requirements
  • Pay Rate: $70-75/hr
  • Start Date: Asap
Location:
  • Field-based, covering Central Metro B (Aurora, Montgomery, Oswego, Yorkville, Plainfield, and surrounding areas)
Job Description:
We are seeking a Home Health RN to manage 20-30 patient visits per week in the field. You will provide skilled nursing care (wound care, IV therapy, ostomy care, etc.) and educate patients on disease management and self-care. All meetings are virtual, and supplies are delivered to you. Must have 1+ year of Home Health RN experience, wound care expertise, and reliable transportation.

Requirements:
  • 1+ year Home Health RN experience
  • Active IL RN license and BLS certification
  • Wound care experience (wound vacs, ostomy care)
  • Proficient in IV and central line care
  • Reliable transportation and active driver's license
