Collaborative interdisciplinary approach supported by nurse case managers, medical assistants, pharmacists, and community resource advisors. Comprehensive, coordinated care including frequent patient visits, chronic condition management, and proactive patient education. Physician role highlights: Provide intensive, personalized care to medically complex patients. Educate and empower patients to manage their conditions effectively. Build lasting relationships and guide patients toward stable health and self-management. Workload and structure: See up to 14 patients per day. Partner with experienced nurses and support staff for seamless care delivery. Ideal candidate: Board-Certified or Board-Eligible in Internal Medicine . Hospitalist experience strongly preferred for managing complex, high-acuity cases. Passionate about chronic disease management and prevention.
