Job Summary:

As a patient navigator, you will play a pivotal role in ensuring a seamless continuum of care and enhancing patient engagement.

Key Responsibilities:

  • Act as a communication bridge among management, administration, healthcare providers, managed care companies, and community organizations.
  • Collaborate with healthcare professionals to promote quality outcomes, streamline treatment processes, and support educational efforts.
  • Track patient outcomes, compile data, and recommend improvements to enhance care processes.
  • Assist uninsured or underinsured patients in finding follow-up care and support services.
  • Utilize nursing processes to assess, plan, implement, and evaluate patient interactions.
  • Help medical providers process diagnostic results, focusing on important findings for patient care.
  • Administer medications while educating patients about therapy, including potential side effects and interactions.
  • Foster relationships by teaching patient education protocols and offering family counseling.
  • Maintain a safe and alert working environment.
  • Perform other related duties as assigned.

Education Requirements:

  • Graduation from an accredited nursing program or equivalent educational background for licensing.

Experience Requirements:

  • Minimum of three years of experience as a registered nurse.

Physical Requirements:

  • Regularly engage in verbal communication, standing, and walking.
  • Occasionally lift and carry items weighing between 25 and 50 lbs.
  • Utilize vision and hearing effectively for patient evaluations and care.
  • Occasional driving required.

Licenses/Certifications:

  • Basic Life Support HealthCare Provider (BLS HCP) certification from the American Heart Association.
  • Registered Nurse (RN) license as per your state of work location.

Work Schedule:

Standard Day Shift; 40 hours per week.
