TLC Nursing Associates, Inc. is seeking a compassionate and reliable Licensed Practical Nurse (LPN) to provide skilled nursing care in a home health setting. This role focuses on delivering high-quality, patient-centered care while supporting patients and families in the comfort of their homes.
Job Responsibilities
- Provide direct nursing care to patients in their homes under the supervision of an RN or physician.
- Administer medications, injections, and treatments as prescribed.
- Monitor patient health status, record vital signs, and report changes in condition to supervising clinicians.
- Assist with wound care, catheter care, and other specialized nursing procedures.
- Educate patients and families about disease management, medications, and home care techniques.
- Ensure proper documentation of patient care activities in compliance with regulatory standards.
- Collaborate with interdisciplinary healthcare teams to coordinate care plans.
- Uphold infection control, safety, and confidentiality standards at all times.
- Current Licensed Practical Nurse (LPN) license in good standing.
- BLS certification required.
- Previous experience in home health, long-term care, or medical-surgical nursing preferred.
- Knowledge of nursing principles, home health procedures, and patient care standards.
- Strong communication, organizational, and patient education skills.
- Ability to work independently while maintaining clinical accuracy and professionalism.
