POSITION PURPOSE

Provides diabetic education for staff, inpatients and outpatients as identified. Follows established policies and procedures of hospital. Collaborates with coordinator and manager regarding instructional activities as necessary.

MINIMUM EDUCATION REQUIRED
  • Bachelor degree: Nursing
MINIMUM EXPERIENCE REQUIRED
  • 3-5 years of medical/surgical nursing experience
  • 2-3 years of experience in diabetes education or related field preferred
LICENSES / CERTIFICATIONS REQUIRED
  • Certified in Diabetic Education preferred
  • Licensed to be eligible to practice as a Registered Nurse in the state of Wisconsin
SUPERVISES
  • N/A
KNOWLEDGE, SKILLS & ABILITIES REQUIRED
  • Certified Diabetes & Education Specialist (CDCES) credential upon hire or within 2 years of hire
  • Must be able to read, write, hear, speak and comprehend the English language
  • Ability to work independently and as part of a team
  • Strong organizational and time management skills
  • Excellent problem-solving and critical thinking abilities
  • Demonstrated teaching ability
  • Experience working with diverse patient populations
  • Proficiency in electronic health record systems
  • Experience in public speaking or group facilitation
  • Knowledge of current diabetes management technologies and devices
  • Knowledge of diabetes pathophysiology, treatment, and management
  • Commitment to patient-centered care and continuous improvement
PRINCIPLE ACCOUNTABILITIES AND ESSENTIAL DUTIES
  • Provides comprehensive diabetic teaching and assists staff with comprehensive diabetic patient teaching
  • Teaches patients on an individual basis or as a group per provider's order
  • Prepares diabetic educational materials for compliance with regulatory bodies and provides outlines and teaching materials for diabetic teaching
  • Develops personalized care plans for patients with diabetes and advocates for patient's needs within the healthcare system
  • Provides instruction to patients on topics such as: Self-monitoring of blood glucose levels - including glucometer use and continuous glucose monitoring systems; How to administer insulin and other diabetes medications - including insulin pump therapy; Nutritional counseling and meal planning guidance; and The importance of physical activity and exercise
  • Assesses patient's knowledge and skills related to diabetes management
  • Supports patients in setting and achieving diabetes-related goals and monitors and document patient's progress over time
  • Collaborates with other healthcare professionals to ensure comprehensive care
  • Remains current in the latest diabetes management guidelines and research
  • Accesses, retrieves and utilizes information from the electronic health record
  • Provides resources and support for managing diabetes-related complications
  • Conducts community outreach and education programs, as requested or assigned
  • Participates in professional development and continuing education activities
