This role is ideal for a Registered Nurse who is passionate about supporting elders and adults with disabilities through direct, community-based care. You will serve as a clinical leader in care coordination, working to assess needs, guide care plans, and empower families through education and support.

  • Conduct in-home visits to assess safety, monitor health status, and provide clinical coaching, including medication reviews.
  • Follow a person-centered care plan and deliver coaching using digital tools, while providing resource referrals and promoting best practices.
  • Build strong, trusting relationships with caregivers to foster meaningful changes and reduce caregiver burnout.
  • Maintain accurate, timely documentation to meet compliance standards and uphold regulatory guidelines.
  • Participate in additional assignments and special projects as required to support team goals and patient outcomes.
Candidate requirements Active RN License in the state of Massachusetts. Minimum of 2 years of recent experience providing direct care to elders or adults with disabilities. Valid driver's license and auto insurance; must be able to travel within the assigned region. Experience in case management, care planning, or assessments, particularly in eldercare or managing complex needs. Comfortable using MS Office, web-based applications, and case management software. Bilingual skills are a plus and may influence compensation. Self-motivated, organized, and able to work independently in the field. Strong interpersonal and communication skills with a collaborative mindset.
