Summary:
The Transitions of Care (TOC) Pharmacist supports safe and effective medication use across inpatient and outpatient settings. This role primarily focuses on completing admission medication histories for hospitalized patients and providing medication therapy management (MTM) services in an outpatient clinic. The TOC Pharmacist also provides cross-coverage for essential inpatient pharmacist functions as operational needs and schedule require.

Schedule:
Day/evening shift

Every 3rd weekend (6-8 hour day shift)

Essential Duties & Responsibilities:

Process medication orders correctly & timely

Lack of QIO reports or complaint, no evidence of errors

Obtain and document accurate admission medication histories within required timeframes

Date/time of intervention logged

Review and resolve medication reconciliation discrepancies within 24 hours of admission

Supervise pharmacy techs & check technician work correctly & in a timely manner

Lack of negative feedback from techs or other departments, no evidence of errors

Deliver needed medications to other departments in a timely manner

Lack of complaint by other departments

Conduct comprehensive and targeted medication reviews in the clinic setting

Date/time of intervention logged

Provide patient education and counseling

Absence of reported issues/incidences.

Provide drug information and clinical support to medical and nursing staff

Lack of negative feedback from techs or other departments, no evidence of errors

Prioritize workload to adhere to budgeted hours

Leaving on time at least 90% of the time (allowing for occasional late day orders)

Report to work on time

Arrives and punches in within 7 minutes of scheduled start time at least 97% of days scheduled. Adheres to OMC absenteeism policy.

Assist in inventory management

Decreased outdating or expired medications in inventory

Assist in new employee training

Time spent training new employee

Maintains open communication with manager

Consistently approaches manager with comments and concerns, manager hears concerns directly

Provides annual self-assessment

Completed forms turned in at review time

Completes required facility education

Healthstream requirements completed, mandatory sessions attended

Maintains licensure, completes required CE

Current license posted in department

Ensures accurate charging of medications/products to patients and/or departments

Lack of identified charging errors

Works independently, performs essential duties of pharmacy manager in his/her absence

Appropriate duties dealt with in manager's absence, appropriate duties saved for manager's return

Assigned project work

Willingness to accept projects, completion of projects

Adhere to state and federal legal requirements

Follow laws pertaining to pharmacy

Qualifications:
  • BS or PharmD degree from an accredited school of pharmacy
  • Competent in use of supplies provided by Osceola Medical Center
Preferred Experience:
  • 1-3 years hospital pharmacy and/or ambulatory pharmacy (MTM) experience.
