We are looking for an experienced Registered Nurse (RN) Case Manager to join our growing team. If you're ready to work for an agency where your voice matters and your work truly makes a difference, we'd love to meet you.
Your Role
- Provide skilled, one-on-one nursing care in patients' homes.
- Complete thorough OASIS assessments and design individualized care plans.
- Monitor progress, document accurately, and coordinate with the interdisciplinary team.
- Educate patients and families on care, medications, and self-management strategies.
- Collaborate with physicians, therapists, and aides for seamless care delivery.
- Participate in case conferences and quality improvement initiatives.
- Active RN license in Nevada
- Minimum 4 years total nursing experience
- At least 2 years in a Medicare-certified home health agency
- Proficient in OASIS assessments and Medicare documentation
- Strong clinical judgment, communication, and organizational skills
- Reliable transportation with valid driver's license and current auto insurance
- Familiarity with EMR systems like Axxess (preferred)

Compensation: $80,000.00 - $120,000.00 per year
