Job Summary:
The Home Care Aide is responsible for providing compassionate services to patients in their home environments.
Key Responsibilities:
- Deliver direct patient care as per individual client needs.
- Assist with personal hygiene tasks including bathing, grooming, and oral care.
- Support mobility needs through transfers, repositioning, and ambulation assistance.
- Carry out basic nursing responsibilities and prescribed exercises as directed by a supervising nurse or therapist.
- Prepare nutritious meals based on individual dietary preferences and restrictions.
- Engage patients in companionship activities tailored to their interests.
- Perform light housekeeping duties as needed.
- Collaborate with team members to maintain high standards of care.
- Document and maintain accurate records required by the agency.
- Report any changes in clients' physical or emotional health to appropriate personnel.
- Utilize the Electronic Visit Verification app and/or required time sheets for task completion.
- Exhibit reliability by arriving on time for all assignments.
- Respect and adapt to the values of clients and their families.
- Encourage independence according to each client's capabilities.
- Follow the care plan and refrain from administering medications.
- Immediately contact Action Health Staffing regarding any concerns, including potential abuse or unsafe environments.
Equipment Used: Transportation, Blood Pressure Cuff, Stethoscope as per the care plan.
Physical demands include the ability to lift approximately 50 lbs. and handle various patient conditions and family dynamics. Assignments are based on availability and caseload; hours per week are not guaranteed. Travel is required within a designated area.
Qualifications:
- Maintain certification with the Department of Financial Services.
- A high school diploma or GED is preferred.
- Strong communication skills; ability to read, write, and follow instructions is essential.
- Physical ability to perform tasks necessary for client care, including lifting and standing.
- Display a compassionate attitude towards caring for the sick.
