Description:
The Clinical Care Team will take referrals from primary care providers.
The team will work with the primary care team to accomplish various tasks.
Responsibilities include social support navigation for social determinants of health such as food insecurity and housing insecurity.
The role involves compiling and maintaining a resource list for social determinants of health resources, including eligibility criteria, referral processes, and contact information.
Collaboration with the primary care nurse and providers is essential.
In-person or remote social needs screenings and assessments will be provided for primary care patients referred by nurses or providers.
Coordination and awareness of social services resources such as housing, clothing, food, and mental health services will be necessary.
Collaboration with other social workers to identify patient and community resources is expected.

The position will require conducting case management activities.
This includes working with hospitals for discharge planning, follow-up, and education.
Assistance in obtaining patient records from hospitals will be part of the role.
The social worker will help secure needed medical equipment through community partners.
Following up on care plans and identifying patients who are lost to follow-up or overdue for care is critical.

Additionally, the role may involve assisting with specialty referral navigation.
Scheduling, coordinating, and tracking specialist and imaging referrals will be required.
Obtaining patient records from specialists and imaging centers is part of the duties.
Maintaining a resource list for specialty referrals, including eligibility criteria and referral processes, will also be necessary.

Assistance to patients in locating and accessing low-cost prescription options is expected.
This may include helping with patient assistance program applications and serving as a patient-provider liaison with drug companies.
The social worker will assist patients with applications for various programs.

There may be opportunities to help with regional primary care-based initiatives that have a social work component.
Documentation in patient records, updating consults, and tagging providers or clinical staff as needed is required.
Providing patient education or finding appropriate education resources will be part of the responsibilities.

Expectations may include completing onboarding and orientation.
Participation in regional office and primary care clinical meetings may be requested.
Attending provider meetings and community meetings to build relationships with social service agencies is important.
The social worker will identify barriers to care or assistance experienced by patients and seek ways to address them.
