Are you a passionate and skilled Physical Therapist looking for a dynamic role where you can make a real difference in patients' lives across different settings?

A dedicated and adaptable Physical Therapist is needed to join the team, providing exceptional patient care at two skilled nursing facilities. The facilities are located in Endicott and Vestal, NY and are 5 miles apart.

In this role, there is an opportunity to work with diverse patient populations and diagnoses while collaborating with dedicated healthcare teams at both facilities.

Responsibilities
• Conduct comprehensive evaluations and assessments, and develop individualized treatment plans for patients with a wide range of conditions, including orthopedic injuries, neurological disorders, and post-surgical rehabilitation.
• Ability to work independently and as part of a team, demonstrating adaptability and flexibility in a multi-site environment.
• Implement therapeutic exercises, manual therapy techniques, modalities, and other evidence-based interventions to improve patient mobility, strength, function, and reduce pain.
• Educate patients and their families on proper exercise techniques, home exercise programs, and adaptive equipment use.
• Maintain accurate and timely documentation of patient evaluations, progress notes, and discharge summaries, adhering to all regulatory guidelines.
• Collaborate with physicians, nurses, occupational therapists, and other members of the interdisciplinary team to ensure coordinated and comprehensive patient care.
• Communicate effectively with patients, families, and colleagues, fostering a positive and supportive environment.

Qualifications
• Graduate of an accredited Physical Therapy program (DPT or Master's degree).
• Current Physical Therapist license in New York State
• Proficiency in Net Heath Software preferred
• Experience in skilled nursing facilities preferred
