Mental Health Rehabilitation Coordinator

- Hourly Wage: $24.00/hr. Part time benefits such as paid time off and paid sick time
- Shift: Part-time (3 shifts per week)
- Schedule: PM shift 3:00 pm-11:30 pm with an every other weekend rotation (Sat & Sun)
- NOTE: Your first day will be spent in orientation from 10:30am to 4pm. The following 4 days will be spent in departmental training from 9am-5pm. After the first 5 days, you will move to the schedule above.
Frankfort Terrace is an Intermediate Care Facility (ICF), providing care to the mentally ill/behavior health population (schizophrenic, bipolar, major depression...). Located in a quiet neighborhood with beautiful grounds, trees and flowers - you can ride your bike to work. What makes us unique:

In other centers, this job may be titled as a Mental Health or Psychosocial Rehabilitation (PSR Specialist), or even a Behavioral Health Rehabilitation Coordinator. Whatever you want to call it, your work here will make a difference.
Qualifications
Psychiatric Rehabilitation Services Coordinator (PRSC):
What we need from you:
- Prior experience as a PRSC or mental health professional in a licensed nursing home, intermediate care facility, or specialized mental health rehab facility is preferred
- Bachelor's Degree in a human services field (including but not limited to sociology, special education, rehabilitation counseling or psychology) and have a minimum of 1 year of supervised experience in mental health or human services is required
- CADC certification helpful
- Experience with Point Click Care (PCC)
- Ability to successfully pass a pre-employment physical and criminal history background check

These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
