As a Home Health Occupational Therapist, your responsibilities will include:

  1. Assessing and screening patient's daily living and work-related skills, and creating therapeutic retraining programs with measurable objectives.
  2. Administering and interpreting diagnostic and prognostic tests to evaluate patients’ functional levels in their home settings, aiding physicians in their assessments.
  3. Collaborating with physicians and other healthcare team members to develop and revise individualized Plans of Care Treatment.
  4. Providing therapeutic treatment and instruction based on physician orders to improve strength, coordination, range of motion, and overall function or to teach compensation strategies.
  5. Reviewing and expanding retraining programs to align with each patient's behavioral goals, while adhering to the overarching rehabilitation program guidelines.
  6. Consulting with vocational team members, as necessary, to craft and implement vocationally-oriented plans tailored to patients’ needs and capabilities.
  7. Reporting on patient status and progress to physicians, rehabilitation staff, Clinical Managers, and family members.
  8. Participating in care coordination and discharge planning activities, acting as a resource for other healthcare personnel to meet patients' needs.
  9. Designing community reintegration activities to support clients in physical reconditioning and psychological adjustment, coordinating these plans with the interdisciplinary team.
  10. Recommending or designing special adaptive equipment to enhance clients’ home or work environments or to improve their engagement in rehabilitation and community activities.
  11. Training staff and family members to implement retraining programs encompassing dressing, feeding, grooming, and hygiene skills, while participating in team and family educational meetings.
  12. Coordinating, overseeing, instructing, and evaluating the performance of Occupational Therapy Assistants and Home Health Aides in delivering occupational therapy services.
  13. Documenting patient care observations, interventions, and evaluations thoroughly on the same day services are provided. Accurately recording any interim verbal orders from physicians and ensuring they are implemented. Submitting evaluations, treatment plans, and discharge summaries to supervisors and care management staff.

Required Experience/Skills:

  • Degree from an accredited Occupational Therapy Program.
  • A minimum of six months of occupational therapy experience is preferred.
  • Experience in Home Health is a plus.
  • Current and unrestricted OT licensure.
  • Current CPR certification.
  • Strong organizational and communication skills.
  • A valid driver's license, auto insurance, and reliable transportation.

Pay Range: $49.00 - $69.00 per visit/unit; annual base pay ranging from $77,200 to $106,200.

Scheduled Weekly Hours: 1

The compensation range provided reflects a good faith estimate for full-time (40 hours per week) positions at the time of posting. Actual pay may differ based on geographic location and individual qualifications including job-related skills, knowledge, experience, education, certifications, etc. Additional compensation range: $85,400 - $117,500 per year.
