Momentum HCS is seeking a dedicated Nurse Practitioner or Physician Assistant to join our Primary Care Clinic in Mountain Home, AR. This outpatient role provides the opportunity to manage an established patient panel within a collaborative, multidisciplinary team of physicians and APPs, delivering high-quality care to the local community.
Clinic Hours:
Monday-Friday
Outpatient-focused with no weekend clinic responsibilities
Team Structure:
Multidisciplinary team of physicians and APPs
Established patient panel with strong community demand
Facility Overview:
This facility is an independent, not-for-profit 268-bed acute care hospital serving a 12-county region in North Central Arkansas and South Central Missouri. We provide care to a service population of approximately 233,000, supported by over 2,000 employees and a robust volunteer auxiliary. Since opening in 1963, our medical staff has grown to include approximately 145 physicians across 32 specialties.

Qualifications:
Licensed Nurse Practitioner or Physician Assistant with current AR state licensure
Experience in primary care preferred but not required
Strong clinical assessment, diagnostic, and patient education skills
Ability to work collaboratively with a multidisciplinary care team
Responsibilities:
Provide comprehensive primary care services to an established patient panel
Conduct evaluations, develop treatment plans, and manage follow-up care
Collaborate with physicians and care team members to ensure coordinated, high-quality care
Educate and counsel patients on preventive health, chronic disease management, and wellness
Maintain accurate and timely documentation in compliance with hospital policies
