Annual Pay Range: $63,860-$64,890
Duties & Responsibilities
- The Assessment Coordinator is responsible for ensuring that youth and families referred to the agency are assisted through the authorization process with the Statewide Management Organization (SMO).
- For families that are deemed eligible for Coordinated System of Care services, the Assessment Coordinator will track the authorization process through the duration of client's enrollment. The Assessment Coordinator will also refer non-eligible families to Directors for assistance with non-CSoC service options.
Complete individualized assessments of each youth referred for Wraparound Process. - Ensures that services are provided in accordance with funding guidelines and are based on the specific needs of children, youth and families.
- Ensures that families are offered choice of either institutional or home and community-based waiver services using the form LTC/CS (Long Term Care/Choice of Services). Ensures family participation and opportunities to identify appropriate goals, strengths, and needs in the development of risk assessments, crisis plans and plans of care.
Qualifications
- Master's degree from accredited university or college in human services, social or behavioral science required. License as a Mental Health Professional is required.
- License as a Mental Health Professional ( LCSW, LPC, LMFT ) is required.
- Three years of experience in the human services field required, preferably with different systems, including schools, behavioral health, child welfare, juvenile justice, health, and delinquency population.
- Must maintain CANS certification through SMO or state training.
- Governance Board and Office of Behavioral Health, including all Child and Adolescent Needs and Strengths (CANS) training.

About our Program:
Eckerd Connects' Wraparound Agencies are part of the Coordinated System of Care (CSoC) under the state's behavioral health managed care program that serves children and adults in Louisiana with behavioral health, mental health, and addictive disorders. Coordinated Systems of Care are national evidence-based models for coordinating services in a family-driven manner to ensure more positive outcomes for children, youth, and families. The model incorporates a broad array of flexible effective services and supports that is organized into a coordinated network and that integrates care planning and management across multiple levels and systems. Through this coordinated network, meaningful partnerships are built with families and youth at service delivery, management, and policy levels. Family-driven and youth-guided services are provided in the home as well as in community settings that are data-driven and outcome oriented. Treatment plans are individualized and strength-based, culturally, and linguistically competent, and integrated across all child-serving systems.
