The Associate Clinical Director will play a crucial role in supervising and training primary care providers (PCPs) in their center. You will be responsible for enhancing our clinical model by improving quality, efficiency, outcomes, and satisfaction for both clinicians and patients. This position includes a blend of direct patient care as a PCP and leadership responsibilities, which will depend on the size and needs of the center.

Key Responsibilities:

  • Provide comprehensive care for patients with both acute and chronic illnesses, particularly in the older adult population.
  • Take full responsibility for patient outcomes and work collaboratively with specialists as needed.
  • Engage consistently with the hospitalist team for patients hospitalized, ensuring continuity of care.
  • Oversee the assessment, diagnosis, treatment, and care coordination for patients with complex health needs.
  • Lead a care team that includes medical assistants, care facilitators, and care coordinators.
  • Coordinate care for patients who are unable to come to the office, including those in hospitals or homebound.
  • Facilitate weekly Super Huddles and Transforming Care Meetings, stepping in for other clinical leaders as necessary.
  • Actively participate in the management of the center, contributing to patient recruitment and staffing needs.
  • Mentor and coach PCPs to ensure outstanding clinical outcomes.
  • Work closely with the Center Operations Director to improve financial performance and increase center membership.
  • Perform additional duties as assigned.

Qualifications:

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty is required.
  • Current and active medical license in your state required.
  • At least one year of clinical experience in a geriatric, adult, or family practice setting preferred.
  • Completion of Chen Medical training and Masterful Conversations program as part of your growth.
  • Board certification in Internal Medicine, Family Medicine, Geriatrics, or related specialty preferred; Board Eligibility required.
  • Must maintain DEA certification for controlled substances.
  • BLS certification required within the first 90 days of employment.

Compensation:

Salary: $231,876 - $331,251
