Palliative Care Social Worker

Wage Range: $38.20 - $63.03 per hour

Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Some positions also offer additional premiums based on shift, certifications or degrees. Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity.
Job Summary:
Responsible for providing and coordinating palliative care services for patients enrolled in the Palliative Care program
Primary Duties:
- In response to consultation requests, assesses, with special emphasis on psycho-social needs, patient suitability for palliative medicine services.
- Makes appropriate referrals to alternate services when palliative medicine is not selected.
- In partnership with the palliative medicine team, participates in the interdisciplinary care planning meetings.
- Available as a consultant to patients, families, staff (hospital, HH and Hospice) and community regarding medical decision making, end of life issues and palliative medicine.
- Assists with providing education for physicians, staff, patients, families and community in palliative medicine.
- Performs other duties as assigned.
License, Certification, Education or Experience:
REQUIRED for the position:
- Master's degree in social work (MSW)
- One of the following credentials is required: Licensed Independent Clinical Social Worker (LICSW), Licensed Advanced Social Worker (LASW). Social Worker Associate License- (LSWAA) or (LSWAIC), Extended education in end-of-life care.
- Demonstrated knowledge and extensive experience with home care, hospice, palliative medicine, advanced directives, pain and symptom relief and discharge planning.
- Current Washington State Driver's license and proof of insurability
- Current Healthcare Provider BLS - American Heart Association by start date
- 3 years of hospice, home health, or palliative care experience
