Job Summary
Children's Memorial Hermann is seeking a dedicated Pediatric Integrated Pharmacist to become part of our incredible team during day shifts. This role is essential in providing superior pharmaceutical care within our pediatric population.
The ideal candidate will have a PGY-1 residency, and a PGY-2 or pediatric experience is greatly esteemed. Responsibilities include filling and labeling prescriptions, delivering drug-related information to patients, hospital employees, and medical staff, and overseeing the pharmacy technical support staff's activities.
Schedule: Day shift, either from 7:30 AM to 4 PM or 8 AM to 4:30 PM, along with every fourth weekend.
Children's Memorial Hermann Hospital is recognized for outstanding quality in multiple pediatric subspecialties, ranking in the top 40 for Pediatric Cardiology and Heart Surgery, Pediatric Gastroenterology and GI Surgery, Pediatric Neurology and Neurosurgery, Neonatology, and Pediatric Orthopedics.

Minimum Qualifications
- Education: Graduation from an accredited School of Pharmacy.
- Licenses/Certifications: Registered Pharmacist licensed in the State of Texas; required Pharmacy Sterile Product certification if compounding sterile products.
Principal Responsibilities
- Fill and label prescriptions while providing drug-related information and education for in-patients and out-patients.
- Dispense controlled substances while maintaining appropriate inventory records.
- Supervise the activities of pharmacy technical staff and mentor pharmacy students or residents.
- Participate in the Medication Use Evaluation Program and Process Improvement initiatives.
- Maintain and review medication profiles for drug-related problems.
- Schedule and prepare IV solutions and additives as needed.
- Adhere to laws governing pharmacy practice and maintain records as required per hospital policies.
- Report medication errors or adverse drug interactions.
- Assist with inventory control and procure out-of-stock items when necessary.
- Engage in clinical interventions, including medication profile reviews, IV to oral conversions, renal dosing adjustments, and therapeutic drug monitoring.
- Document clinical interventions accurately.
- Ensure patient, staff, and visitor safety by adhering to all Memorial Hermann policies and procedures.
- Promote professional growth through education and mentorship of less experienced staff.
- Demonstrate a commitment to compassionate, personalized caregiving that adheres to high service standards.
- Perform other duties as assigned.
