Responsibilities:

  • Provide direct support with daily activities, including bathing, dressing, grooming, and eating to promote health and independence.
  • Assist with mobility and functional nursing care as per the nursing care plan, including transferring and positioning.
  • Build professional relationships with residents, families, and staff to foster open communication.
  • Assist with assessments and treatment procedures as outlined by the State Nursing Practice Act and facility policies.
  • Follow infection control protocols to ensure a safe environment.
  • Support residents with nutritional and hydration needs while encouraging engagement in social activities.
  • Report any changes in residents' physical or mental conditions to the nurse leader.
  • Document the level of care accurately, including LNA flow sheets and other required data.

Qualifications:

Active LNA License in New Hampshire is required.

Prior experience with senior citizens is highly preferred.
