Marketing Community Liaison

Job Description
As the home health community liaison Primary responsibility is to develop and maintain relationship with referral sources like physician hospitals skilled nursing facilities assisted living communities and other health care providers. The role focuses on developing and maintaining relationships and building long term partnerships with community.
Primary Responsibility:
- Serving as a liaison between referral sources and clinical team to ensure seamless communication and quality patient care
- Implement strategic marketing plans to generate new referrals and growth including patient senses
- Establish new relationships with referral sources like physician officers karma case managers discharge planners and social workers
- Maintain new relations by conducting regular visits, presentation in services and educate potential referral sources about home health services
- Work with clinical team and administrative team to develop seamless transition of patients from facility to home.
- Represent agency at community events health fairs and professional networking meetings and collaborate with clinical team to ensure high quality service and patient satisfaction
- Maintain compliance with all state and federal regulations related to marketing patient privacy like HIPAA and ethical regulations.
Preferred Experience:
Previous experience with Medicare Certified home health, hospice or home care is preferred with existing network of healthcare contacts in local community
Compensation : Excellent salary with bonus structure which exceeds industry standards.
