JOB DESCRIPTION

McLaren Home Health delivers clinically excellent, compassionate home health care services to patients in the comfort of their homes, including skilled nursing visits, wound care, infusion therapy, and more. 

As a Registered Nurse - Home Health, you will:

Provide medically prescribed skilled nursing care, health counseling, and support to patients in their homes
Perform initial patient assessments and develop individualized care plans based on physician orders
Deliver essential treatments, monitor patient status, and educate patients/caregivers on health management
Document care, communicate changes in condition, and coordinate with interdisciplinary care teams

This role typically involves a caseload of approximately 7 visits per day, using Home Care Home Base EMR.
