Child Welfare Services Case Coordinator

At LSF, we are seeking dedicated Child Welfare Services Case Coordinators to make a significant impact in the lives of children and families.
Purpose & Impact:
The Child Welfare Services Case Coordinator plays a vital role in delivering comprehensive case management services to LSF clients, building strong relationships while embodying the agency's core values.
Essential Functions:
- Accept cases assigned by the unit supervisor and conduct initial visits with children in their homes within 2 working days of assignment, completing a Family Assessment within 15 working days.
- Negotiate and create a case plan collaboratively with families, focusing on identified strengths, needs, and the ultimate goal of reunification or other permanency for the child.
- Regularly visit children in their homes with a minimum frequency of once every 25 days, ensuring at least one unannounced visit each quarter.
- Document all case activities in the Florida Safe Families Network within 48 hours and maintain organized physical case files.
- Submit service requests to the Utilization Management Department and provide timely referrals to community services, maintaining knowledge of available resources.
- Prepare and file court documentation in a timely manner and actively participate in court proceedings as necessary.
- Conduct ongoing child safety assessments and develop safety plans as applicable.
- Conduct diligent searches for parents and family members when necessary, documenting all efforts thoroughly.
- Facilitate home studies for prospective placements, providing background information about children to caregivers.
- Transport children as needed and fulfill all Independent Living requirements.
- Engage in continuous professional development by attending all required meetings and training sessions.
- Work collaboratively with various stakeholders, including Program Directors, Case Managers, legal staff, and other support personnel.
- Manage time effectively to ensure completion of home visits, documentation, and court responsibilities on schedule.
- Follow all relevant regulations, laws, and agency policies to manage cases towards successful outcomes.
- Prioritize and complete all work assignments by established deadlines.

- Training: Participate in and complete all required training courses and assessments.
- Confidentiality: Maintain strict confidentiality regarding all client information.
Qualifications:
Physical Requirements:
- Must have a valid Florida Driver's License, insurance, and a reliable personal vehicle for transportation.
- Willingness to travel locally and beyond for professional duties.
- Ability to use technology effectively and maintain accurate written records.
- Flexibility to work irregular hours and be available for on-call responsibilities.
- Certification in First Aid/CPR and crisis intervention is required.
Education/Experience:
- A minimum of one year of relevant experience is required, with certification needed within one year.
- A Bachelor's degree in a Human Services field is required; a degree in Social Work is preferred.
Skills/Abilities:
- Strong written and verbal communication abilities.
- Proficient in Microsoft Office applications, including Word and Excel.
- Ability to travel as necessary for case management duties.
- Understanding of case management responsibilities and sensitivity to the cultural and socioeconomic needs of families served.
