Outpatient Therapist/Social Worker needed in New Castle County, Delaware.
Join an intensive outpatient program dedicated to supporting participants recently discharged from inpatient care. Our mission is to help clients avoid rehospitalization, manage crises effectively, and achieve sustainable recovery within a compassionate and supportive environment.
- Develop and implement individualized treatment plans
- Provide therapeutic counseling to build coping skills and resilience
- Collaborate with a multidisciplinary team to ensure continuity of care
- Monitor and adjust treatment plans to reinforce recovery
Position Requirements
DE LMSW or LCSW with prior outpatient or related mental health experience preferred.
