This is a salaried,in-personposition based at79 Bramhall Street in Portland, Maine,supporting both in-patient services at Maine Medical Center (MMC) and outpatient oncology practices. The role is part of a dedicated team that operates Monday through Friday between 7:00 AM and 5:30 PM, with a rotating Saturday coverage schedule.

In this role, team members meet with patients and their families to assess eligibility for a wide range of community, state, and federal programs. These include Medicaid, Marketplace insurance, SNAP, PHIP, long-term care, disability, Medicare, and Free Care (MaineHealth Charity Care). Staff provide comprehensive, hands-on support throughout the entire application process, ensuring patients have access to the resources they need. The team also plays a vital role in discharge planning by submitting long-term care applications for patients who will require placement in a facility after hospitalization.

Team members meet withpatientsbedside at Maine Medical Center and support outpatients at the South Portland and Scarborough oncology practices.
Travel between these locations is required.

Summary:
The Case Manager - Medical Outreach role is responsible for the provision of case management services and achievement of positive intervention outcomes for assigned clients. They work on-going with patient caseloads, delving deeper into more medically and socially challenging issues and health needs, addressing barriers for patients.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: Bachelor's Degree preferred, with a specialization in Psychology, Behavioral Health, Social Work, Special Education, Counseling, Rehabilitation, Nursing, or closely related field preferred.
  2. License/Certifications: Current applicable state(s) license as a Licensed Social Worker preferred. Valid driver's license and reliable vehicle required.
  3. Experience: With a bachelor's degree: Two years of experience in a community health setting, including one year of case management experience required. In lieu of bachelor's degree: Three years of directly relevant experience in a healthcare setting, including 6 months of case management experience. Experience collaborating with state and federal agencies, community agencies, municipal, and public programs preferred.
