Licensed Practical Nurse (Skilled Nursing Facility)

We are seeking a dependable and compassionate Licensed Practical Nurse to provide quality nursing care to residents in a skilled nursing facility. The LPN will assist with daily clinical tasks, support resident care plans, and ensure a safe and comfortable environment while working collaboratively with the healthcare team.

Responsibilities:
  • Provide direct nursing care, including routine assessments, treatments, and medication administration.
  • Monitor residents' conditions and promptly report changes to the supervising nurse or provider.
  • Support care plan implementation and assist with documenting resident progress.
  • Collaborate with RNs, CNAs, and other staff to deliver resident-centered care.
  • Maintain accurate and timely nursing records in accordance with facility guidelines.
  • Promote a safe, respectful, and supportive environment for residents and families.
Qualifications:
  • Current LPN license (active and unrestricted).
  • Experience in skilled nursing or long-term care preferred.
  • Strong communication, organization, and clinical skills.
  • Ability to work effectively as part of a multidisciplinary team.
