As a Home Health Occupational Therapist, your key responsibilities will include:

• Delivering occupational therapy to patients who struggle to perform daily tasks due to an illness or injury.
• Collaborating as part of a home health team, which may include RN, LPN, CNA, HHA, PT, COTA, and SLP, focused on the patient's plan of care and personal ambitions.
• Assessing patients, observing deficits, establishing therapy goals, and documenting progress effectively.
• Guiding patients through exercises aimed at improving fine motor skills and coordination.
• Recommending adaptive equipment such as grab bars and shower chairs for additional support.
• Evaluating fall risks and proposing strategies to enhance home safety.
• Educating patients and families on the care plan, exercises, goals, and self-care techniques.

Key qualifications for Home Health Occupational Therapists:

• Graduation from an accredited Occupational Therapy Program and a valid OT license in Colorado.
• At least two years of relevant experience as an occupational therapist within the last five years preferred.
• CPR Certification (a demonstration course is required).
• Familiarity with state and federal home health regulations.
• Strong clinical judgment, interpersonal skills, resourcefulness, and compassion.
• No COVID vaccination requirements.
