LTC LMSW (Medical Social Worker) - Seattle, WA
Facility is only review travelers who are interested in a temp to perm opp.

- Start Date: ASAP
- Shift: 5x8hr Days (Mon-Fri; Weekends not required)
- Daily Patient Load: 1:50-60+ (1:1 patient care and group counseling)
- Years of Experience REQ: At least 1 YR of experience working with geriatrics / At least 2 YR of experience working with geriatrics from ethnically diverse groups
- First-Timers Accepted: Yes
- Weekend REQ: No
- Certs REQ: BCLS
- Is On-Call REQ?: No
- Will this traveler need to float between like units?: No
- Open to accommodating block schedule?: No
- Locals Accepted: Yes; Considered "local" within 50-75 mile radius, will accept locals at the same rate
- WA license in-hand REQUIRED (at time of signature); Required upon hire: Washington Advanced Social Worker License (Vendor Managed) Or Required upon hire: Washington Advanced Social Worker Associate License (Vendor Managed) Or Required upon hire: Washington Clinical Independent Social Worker License (Vendor Managed) Or Required upon hire: Washington Clinical Independent Social Worker Associate License (Vendor Managed)
- RTO Restrictions: Confirm with hiring manager
- Guaranteed Hours: Per contract, facility can call off UP TO one shift (equivalent total hours) per pay period (every two weeks) if needed

Special Requirements
- Traveler is expected to have their own reliable transportation
- EPIC experience is REQUIRED
- Department Details & Patient Diagnoses: As a member of the interdisciplinary team, provides social work services to participants in a manner which is consistent with the mission and core values of Providence Health System. Spends a majority of time in direct service activities within the Center, including one-on-one and group counseling, interaction with other team members, and ongoing assistance to participants to address social issues. Based on comprehensive assessments, provides perspective on the social, emotional, cognitive and ethnic factors that impact the creation of an individualized integrated plan of care. Accesses community and financial resources for participants and ensures continuity of care across the continuum. Conducts social work assessment and intervention in other care settings, including the participant's home, as needed. **Roughly 60% out in the field seeing patients either in their home, AFH, ALF, or SNF's and 40% onsite in the clinic, but can vary depending on case load.**
