Position Summary

The Hospice Admissions Nurse (RN) plays a critical role in welcoming patients and families into hospice care. This position involves conducting concise assessments recognizing urgent needs and prioritizing what is needed to get the patient and family comfortable and started on their hospice journey. The Admissions nurse is also able to balance multiple admissions, documentation and responsibilities in one day.

Key Responsibilities

  • Conduct timely admissions assessments in homes, facilities, or hospitals.
  • Explain hospice philosophy, services, and benefits to patients and families.
  • Evaluate patient eligibility for hospice services based on clinical criteria.
  • Collaborate with physicians, case managers, and other healthcare providers.
  • Initiate and complete all required documentation accurately and efficiently within EMR systems (eg. MyUnity/Netsmart)
  • Ensure a smooth transition into hospice care with compassion and professionalism.
  • Maintain compliance with all regulatory and organizational standards.

Requirements

  1. Graduate of an accredited school of nursing. One (1) to two (2) years of recent critical care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care.
  2. Current RN Registered Nurse Licensure in State of Maryland and CPR certification.
  3. Bachelor's degree, with one (1) year of hospice care experience preferred.
  4. Strong clinical assessment and communication skills.
  5. Compassionate demeanor and commitment to patient-centered care.
  6. Ability to work fast paced, independently and efficiently in diverse settings.
  7. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
  8. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
