JOB SUMMARY: Implements outreach/inreach and referral services to provide housing stability to homeless and at-risk veterans and their families.

JOB DUTIES:
  1. Completes intake form with clients to determine eligibility. Distributes literature about the SSVF program and available community resources and benefits of the program to eligible veterans.
  2. Completes initial eligibility disposition form to determine services.
  3. Travels the county to conduct and coordinate participation in street outreach to identify homeless veterans and presents the SSVF Program as an alternative to homelessness.
  4. Refers eligible SSVF clients to services identified within the intake process and provide transportation to veterans and their families as needed to provide these services.
  5. Makes referrals to other community resources and advocates for veterans and their families as needed. Assesses and evaluates barriers to homeless veterans and their families' utilizing resources and determines steps which can be taken to meet and overcome these barriers.
  6. Prepares and maintains an accurate and up to date listing of all known community resources, events, and general information within area. Seeks out new resources as they are established and developed in community. Establishes and maintains good public relations with other veteran's agencies. Makes recommendations for strategies to reach those segments of the population which are not utilizing services. Participates in all Stand Down speaking events to bolster awareness of Catholic Charities SSVF program.
  7. Maintains a daily activity log and completes files on each enrolled SSVF client as required by contract. Compiles data as required and completes reports as scheduled or assigned. Monitors expenditures of assistance funds.
  8. Promote ongoing commitment to excellence and continual quality improvement to ensure the highest quality of services to our staff and clients.
  9. Performs other duties as assigned.
JOB REQUIREMENTS:
  1. Education /Background: Minimum of a High School Diploma or Equivalent plus 1 year of relevant experience. Associates degree preferred. Bilingual (Spanish) preferred. Veteran status is preferred.
  2. Job Knowledge: Must possess good interviewing skills, must be knowledgeable of local and state resources must have knowledge of appropriate contact guidelines and requirements. Must keep accurate and detailed records. Position requires ability to handle a variety of problems and pressures and must be effectively able to handle stress. Must be proficient in Microsoft Office (Word and Outlook).
  3. Working Conditions/Physical Demands: Must be able to stoop, bend, squat, sit on floor and ambulate over uneven floor surfaces (hiking) including stairs and steps. Must be able to visually and auditory assess and interact with clients to ensure their safety and wellbeing. Requires considerable work outside of the office and substantial driving. May lift up to 30 pounds.
  4. Other requirements:
    1. Valid AZ driver's license, reliable transportation and ability to show proof of registration/insurance.
    2. Level One Fingerprint clearance or ability to obtain fingerprint clearance; must maintain clearance.
    3. You must be 21 years of age or older to drive on behalf of Catholic Charities.
    4. Must have or complete within 90 days of hire, training in CPR and First Aid and remain current in the certification

Hourly rate starting at $19.11 per hour and may increase depending on experience.
