Your Role:

  • As a Licensed Practical Nurse reporting to the Coordinator of Care Nurse, you will conduct in-home visits based on patients' skilled needs.
  • You will implement individualized care plans and evaluate treatment effectiveness.
  • Your documentation will be handled electronically, with a commitment to complete it within 24-48 hours.

Qualifications:

  • Current New York State LPN License.
  • At least one year of Med/Surg experience in a hospital or long-term care facility.
  • A valid driver's license and access to a car may be necessary based on job location.
